The last one is the "very special episode". It has ER's "soft" intro, without the percussion at the scene transition.
Very good, fellow ER nerd. Its been a while since I put that clip together, but I think the "very special episode" is Season 2, episode 9 "Home". I might be wrong, if I am, don't just yell at me, just tell me the name of the damn episode!!!
